Erin Simao

Hometown Health Centers

Erin Simao has over 10 years of experience working in Schenectady, NY, leading and managing community health initiatives focused on the underserved. She has worked in a number of different settings; Schenectady County Public Health Services as the Public Health Systems Administrator; Ellis Medicine as the Community Health Outreach Coordinator; Alliance for Better Health as the Transformation Specialist for Cultural Competency and Patient Activation; and City Mission of Schenectady as the Program Manager for Empower Health, a program that conducted outreach and health and social services navigation for Medicaid and the uninsured utilizing a Community Health Worker-inspired model for staffing. Erin is currently the Manager of Practice Transformation for Hometown Health Centers, focused on practice innovation and population health. She completed her MBA in Healthcare Management at Clarkson University in August 2021 and received her Master of Public Health degree from the University at Albany in 2011. Erin also spent 3 years teaching students in the Community Health Worker certificate program at SUNY Schenectady.
