
Sarah Eip
St. Mary's Healthcare
Sarah Eipp is the Executive Director of Population Health and Continuum Services at St. Mary’s Healthcare. She has served in several roles since joining St. Mary’s Healthcare in 2010.
As a Registered Dietitian, Sarah has served as the Clinical Nutrition Manager and Diabetes Center Coordinator. More recently, she has served as the DSRIP lead for St. Mary’s Healthcare as part of the Alliance for Better Health PPS and the St. Mary’s lead for the Innovative Health Alliance of New York’s (IHANY) clinically integrated network. She has also served as the Director of the adult and children’s Health Home programs for St. Mary’s, both of which are designated as Lead Health Homes.
In her current role she serves as the executive lead for several service lines across the care continuum including the Wilkinson Residential Healthcare Facility, Palliative Care, Therapy services, Physical Medicine and Rehabilitation and Clinical Resource Management and works to facilitate a coordinated approach to patient care and population health at both the health system level and with community partners.
Sarah currently represents St. Mary’s on the Healthy Alliance IPA Board of Managers and is a member of the IHANY Joint Operating Committee. She is also a board member for Community Health Center CHHA and Home Healthcare Partners LHCSA both located in Johnstown.
Sarah holds a Bachelor’s degree in dietetics from the State University of New York at Oneonta.
